We are committed to securely collecting, storing, using and protecting your personal information to the extent at which it is necessary for the services provided by our studio.
We may disclose and share your personal information under the following circumstances:
The studio receives a signed Consent to Release Information from the patient and if applicable, their a lawyer’s office;
The Acupuncturists, in a private meeting, are discussing the progress of a patient they are treating;
Communicating with a medical personnel or 911 operator in the event of an emergency.
WHAT PERSONAL INFORMATION IS COLLECTED?
We receive and store two kinds of information:
1) That which you provide on your initial New Patient Intake form, and
2) The notes that acupuncturists make during or after your visit.
Your intake forms are scanned and stored in an encrypted hard drive. Notes from each acupuncture session are recorded in an electronic format and we are mandated to keep them securely for ten years.
WHAT PERSONAL INFORMATION IS COLLECTED BY THIRD PARTIES?
Our studio utilizes three third-party service providers. They are:
Gmail – Scanned files are downloaded onto a laptop through Gmail. Google’s privacy statement can be found here.
Pocapoint – Our online scheduling and electronic health record management software developed by a colleague and web developer in Nashville, TN. Their privacy statement can be found here.
Practice Fusion (From Jan 2014 – Dec 2014 only) – In 2014, we utilized a web-based, electronic health record management program. We are no longer using this program. Their privacy statement can be found here.
